I'm level 41 and don't see a Battlegrounds or PvP really anywhere, is this because the East is more PvE oriented than West?

From 1-43 there really isn't any pvp as the pvp dailies are "group hug", where the factions ignore each other. However, once you get to 43 and are able to do the Misty Forest dailies, there is one pvp daily that requires you to last hit 10 players of the other faction, and that's the main source of PvP in this game right now. I think there is also a PvP arena thats in place in the test servers, which will support 1v1s and 3v3s. The PvP daily quest is mostly a large group of one faction vs. a large group of the other.

Any idea on what endgame PvP gear is like?

For example, is there tiers?

This game is mainly PVE right now (Which is kind of odd, given how insane into PVP Koreans are).
I believe they'll be adding arenas soon and there is their little duel system (Which I think it's arena anyway?). Not sure exactly.

I hope they add some form of battleground before it makes it to NA/EU.

I guess there really isn't set "tiers", but right now the best PvP gear is probably the bopae you get from the 8th floor in the Tower, which gives tons of HP and I think defense. Most people enchant their bopae with Crit-Defense for PvP.
